Repair Policy Reminder

I hereby authorize the above repair work to be done along with the necessary materials required. You and your employees may use the equipment for the purposes of testing and inspection at my risk. An express lien is acknowledged on the above equipment to secure the amount of repairs thereto. Balances not paid within 14 days of the date of repair completed are subject to a finance charge at a percentage rate or 1 3/4% per month (21% per year) applied to the past due balance after deducting current payments and credits. A STORAGE FEE OF $2.50 PER DAY will be charged after 14 days from the invoice date or date of repair completed whichever is later. If this account is placed in the hands of an attorney, then reasonable attorney's fee will be paid by the customer for the prosecution of collection of this account. There is a $30.00 minimum charge for any estimate of repair work to be done. Additional estimate charges may be required for some types of estimates.

Notice: Balances not paid within 14 days of the COMPLETED date of repair are SUBJECT TO A STORAGE FEE OF $3.00 PER DAY after 14 days from the invoice date or date of repair completed whichever is later. If
this account is placed in the hands of an attorney, then attorney's fee will be paid by the customer for the prosecution of collection
of this account. There is a $35 minimum charge for any estimate of repair work to be done. Additional estimate fees may be required on some instruments. AFTER NOTIFICATION INSTRUMENTS WILL BE SOLD AFTER 45 DAYS FROM COMPLETION DATE.
